Requirements: Applicant must apply via postal mail and submit all required information in its entirety when returning the application. Incomplete applications will not be forwarded to the Scholarship Review Board for consideration. Applicant must write an essay on how their diagnosis has impacted their lives and their future goals. Applicant must submit a copy of an acceptance letter form the college, university, or vocational-technical school planning to attend. Applicant must also submit written documentation form your treating physician stating your medical history. Applicant must provide two letters of recommendation from non-related persons. Please ask that each reference include their names, address, and phone number within the letter. Applicant must provide an official high school and/or current college transcript. Must also complete financial form that will demonstrate financial need and include a copy of the first two pages of tax returns.

It is important to investigate the latest information on the scholarship. Your best chance of obtaining money comes from first understanding the qualifications and making sure you meet them. A good first step is to be aware of the reason for involvement of the Patient Advocate Foundation and their goals for the Scholarship for Survivors.
